When did it become status quo to refer to Americans as fat and ignorant?

I don't know. Ignorant can mean different things to different cultures. Could it be the Americans are ignorant of other World cultures? Could it be America has joined the world joke club? I wonder if it would be worthwhile for America to do lots of research into the reasons why people everywhere are fatter today than they were. There are fat people in Britain.
